During Sunday’s game against the Houston Texans, Colts rookie quarterback Anthony Richardson sustained a concussion in the second quarter. The exact moment of Richardson’s injury remains uncertain. Prior to his exit, he demonstrated commendable performance, completing 6-of-10 passes for 56 yards, in addition to carrying the ball for 35 yards and securing 2 touchdowns. Gardner Minshew stepped in for Richardson and effectively led the Colts to a touchdown drive.

For Richardson’s return to play, he must adhere to a comprehensive five-phase protocol.

In the first phase, known as “Symptom-limited Activity,” Richardson is advised to prioritize rest and steer clear of activities that may exacerbate his symptoms. Under the watchful eye of the athletic training staff, he may gradually introduce limited stretching and balance training, eventually progressing to light aerobic exercise. The next phase hinges on Richardson’s ability to engage in cardiovascular exercise without experiencing an escalation of symptoms or any signs of a concussion during a neurological examination.

The subsequent stage, “Aerobic Exercise,” involves Richardson embarking on a program of graduated cardiovascular exercise, accompanied by dynamic stretching and balance training, all under the vigilant supervision of the team’s staff. Only when Richardson can partake in cardiovascular exercise without aggravating symptoms will he advance to the next phase.

In the third phase, termed “Football-specific Exercise,” Richardson continues under supervised cardiovascular exercises, with the intensity potentially mirroring sport-specific activities. Here, supervised strength training is also incorporated into his regimen.

Moving on to the fourth phase, “Non-contact Training Drills,” Richardson maintains his cardiovascular, strength, and balance training. This stage introduces team-based, sports-specific exercises, along with participation in non-contact activities.

The final phase, “Full Football Activity/Clearance,” is the pivotal step. It entails clearance by the team doctor for Richardson to engage in full football activity. Moreover, an examination by the independent neurological consultant assigned to the team is imperative. Only if both the team physician and consultant are in unanimous agreement that Richardson’s concussion has been fully resolved will he be allowed to participate in practice and contact without any restrictions.
