The central government on Monday issued fresh guidelines for the third phase of the COVID-19 vaccination drive across the country from May 1, saying everyone above the age of 18 will be eligible to receive jabs.

This comes with the country witnessing a record surge in COVID-19 cases, over 2.7 lakh new cases on Monday. 

The Centre said the procurement, eligibility and administration of vaccines for the third phase will be made more flexible as it aims to curb the spread of the virus.

If you fall into any of the aforementioned category, here is how you can register yourself to receive COVID-19 vaccination:

Offline/online registration

Registration can be done both online and offline. Those who wish to apply online can do so by logging in to Arogya Setu app or Co-Win app and opt for self-registration option.

For offline candidates, registration windows have been opened in hospitals and Common Service Centres. There will be no charge for registration.

Documents required

For those above 60 years of age will be required to submit a photo ID while for people with comorbidities, a medical certificate will also be required along with the ID.

Others can provide any of the below-mentioned ID with Photo at the time of registration:

-Aadhaar Card

-Driving License

-Health Insurance Smart Card issued under the scheme of Ministry of Labour

-Mahatma Gandhi National Rural Employment Guarantee Act (MGNREGA) Job Card

-Official identity cards issued to MPs/MLAs/MLCs

-PAN Card

-Passbooks issued by Bank/Post Office

-Passport

-Pension Document

-Service Identity Card issued to employees by Central/ State Govt./ Public Limited Companies

-Voter ID

Other formalities

For comorbidity data, the government may issue a form which a prospective vaccination candidate will have to fill the form and get it attested by a medical practitioner after an examination.

Choice of centres

People will be given a choice of vaccination centre. A candidate can opt for a suitable centre near him.

No choice of vaccine dose

Beneficiaries will not be permitted to take a pick of vaccine doses available in India, i.e, no choice between Covishield, Oxford-Astrazeneca vaccine, developed in India by Pune’s Serum Institute, and Bharat Biotech’s Covaxin.
